Rate Per Session

The fee for a 50-minute private-pay session is $150.00.  Cash, local checks, and debit/credit cards are accepted.   A convenience fee of $2.50 is applied for debit/credit payments. In addition, payments may be made by PayPal if you are unable to pay by cash or check.

Note: Co-payments or Co-insurances are due at the time of the appointment.  Please verify benefits and obtain necessary prior authorizations.  Contact the number on the back of your member identification (ID) card for any questions or benefit rules.

Insurances with an out-of-network benefit coverage

If you have insurance with an out-of-network benefit, you can see me. However, I do not submit claims but will provide you the necessary statement/invoice from the session to file with your insurance plan. How out-of-network benefits work is that I will collect payment from you in full at the time the service is rendered and then you will have to send your insurance carrier the claim yourself. 

How to submit a claim for out-of-network services

You’ll need to fill out a claim form and mail it to your health insurance carrier. Then they can figure out what your plan covers and how much. They will reimburse you directly for charges your plan should pay. Please contact your insurance carrier for specific instructions and forms for submitting your claim for reimbursement.
