The short version of the Notice of Privacy Practices (NPP) will require your signature to acknowledge that you received a copy and will remain on file with my office. A longer, comprehensive version of the Notice of Privacy Practices is also available for download and for your personal records.
Please bring the completed documents to your first appointment. It is important that all sections of the forms are completed. If you do not have the ability to print the forms before your initial intake appointment, please arrive 15 minutes early to allow time to complete.
